Proposition 64 Public Health & Safety Grant Program Local Evaluation Plans
Local Evaluation Plans (LEPs) are a mandatory component of the Prop 64 PH&S Grants. These LEPs ensure projects funded can be evaluated to determine their impact and effectiveness and should describe how the project will be monitored and evaluated and shows how evaluation results will be used for project improvement and decision making. There is no formal template for the LEPs; however, the BSCC provides a guideline that, at a minimum, addresses the following:
- Cover Page
- Project Background
- Process Evaluation Method and Design
- Outcome Evaluation Method and Design
- Project Logic Model
- Appendices (if applicable)
In addition, the Prop 64 PH&S Grants are required to submit a 3-Year Local Evaluation Report (LER) to identify whether the project was successful in achieving its goals and objectives identified in their application for funding and LEP. LERs will be posted at the end of each cohort’s contract period.
